Privacy is a fundamental right, recognized in the state and federal constitutions. Since 1983 California’s Confidentiality of Medical Information Act has guaranteed that patients can obtain their medical information from their healthcare providers with limited exceptions. The Privacy Rule permits covered entities to disclose PHI for treatment, payment, and health care operations without the need to first obtain an individual’s authorization or receive an access request by the individual to have the individual’s PHI directed to a third party for such purposes. As a result, if an individual is seeking to have her PHI shared among her treating providers, the covered entities can and should do so; the individual should not have to facilitate this transmission by submitting an access request.
Ampla patients are welcome to request copies of their records from their provider without any barriers. Please follow instructions below:
For Patient Medical/Dental record requests:
For requests that are under 20 pages, no charge
For requests of a record over 20 pages through 129 pages, charges would be $.05 per page.
For requests of a record over 130 pages through 150 pages, charges would be $6.50
Any requests of a record over 150 pages will be referred to Sharecare.
Patient transferring care:
When requests for records comes from another provider, there is no charge for sending the records.
When a third party request comes in (lawyers, courts, other agencies, etc.)

Sharecare Health Data Services is the Release of Information Service for Ampla Health . Once records have been copied you will receive an invoice from Sharecare. Although we cannot provide an exact amount, as rates are based on time and material, here are some examples of cost based on the information requested and media preference:

When will your records be ready?
