No. CCM works alongside your primary care provider to support your healthcare between office visits.
Is CCM covered by Medicare?
CCM is a Medicare-covered service.
Patients with Medicare who do not have secondary insurance may have a monthly copay. If you have Medicare and a secondary insurance, your out-of-pocket cost may vary depending on your coverage.
Can I stop participating?
Yes. Participation is completely voluntary, and you may cancel the program at any time.
How do I enroll?
Talk with your healthcare provider about Chronic Care Management. Your provider can submit a referral to the CCM team.
