What is Chronic Care Management?
Chronic Care Management (CCM) is a Medicare-covered service designed for patients living with two or more chronic health conditions. Through regular communication and personalized care planning, our dedicated care team works alongside your healthcare provider to help you stay on track with your health goals.
How We Help
Our Chronic Care Management team provides ongoing support that may include:
- Personalized care planning
- Medication management and education
- Assistance coordinating care with providers and specialists
- Help scheduling appointments
- Support accessing community resources
- Monthly phone or text check-ins between office visits
- Education to help you better manage chronic conditions
- Answers to your healthcare questions
Do You Qualify?
You may qualify if you:
- Have Medicare or Medicare with a secondary insurance payer
- Have two or more chronic health conditions
- Have conditions expected to last at least 12 months or for the remainder of your life
Common Conditions We Help Manage
Our program supports patients with 100+ qualifying chronic conditions. Common examples include:
- Diabetes
- High Blood Pressure
- Heart Disease
- COPD
- Asthma
- Chronic Kidney Disease
- Arthritis
- Depression
These are only examples. Your provider can determine whether your condition qualifies for CCM services.
What to Expect After You Enroll
After enrollment, your dedicated care team will work with you to help manage your ongoing healthcare needs through non-face-to-face care coordination.
You may receive:
- Monthly phone or text check-ins
- Medication reviews
- Personalized care planning
- Assistance coordinating appointments
- Education and self-management resources
- Referrals to community programs when appropriate
Frequently Asked Questions
Does CCM replace my regular healthcare provider?
No. CCM works alongside your primary care provider to support your healthcare between office visits.
Is CCM covered by Medicare?
CCM is a Medicare-covered service.
Patients with Medicare who do not have secondary insurance may have a monthly copay. If you have Medicare and a secondary insurance, your out-of-pocket cost may vary depending on your coverage. Our team will discuss any potential costs with you before enrollment.
Can I stop participating?
Yes. Participation is completely voluntary, and you may cancel the program at any time.
How do I enroll?
Talk with your healthcare provider about Chronic Care Management. Your provider can submit a referral to the CCM team.
