What is Chronic Care Management?

Chronic Care Management (CCM) is a Medicare-covered service designed for patients with two or more chronic health conditions. Through regular communication and personalized care planning, our dedicated care team works alongside your healthcare provider to help you stay on track with your health goals.

Our Chronic Care Management team provides ongoing support that may include:

  • Personalized care planning
  • Medication management and education
  • Assistance coordinating care with providers and specialists
  • Help scheduling appointments
  • Support accessing community resources

  • Answers to your healthcare questions

Do You Qualify?

You may qualify if you:

  • Have Medicare or Medicare with a secondary insurance payer
  • Have two or more chronic health conditions
  • Have conditions expected to last at least 12 months (or for the remainder of your life)

Frequently Asked Questions

Does CCM replace my regular healthcare provider?

No. CCM works alongside your primary care provider to support your healthcare between office visits.


Is CCM covered by Medicare?

Many Medicare patients qualify for coverage. Depending on your insurance plan, a monthly copay may apply.
